Change of Address

Have you changed your address or phone number recently? Please remember to let us know. We find often that the phone numbers given to us initially when patients register are not up-to-date when we try to make contact.

Out of Hours

During these times if you telephone 0131-663-1051 you will be given information on how to contact the out of hours service.

Appointments

For routine appointments please telephone 0131-663-1051. The receptionist will give you the first available appointment with the doctor of your choice.

Home Visits

The doctors will visit patients who cannot manage to attend the Medical Centre. For routine house calls, please telephone 0131-663-1051 (preferably before 10.00 am) and leave your information with the receptionist.

CARERS

Are you a carer? Or do you think you might be a carer? Do you have someone who looks after you?

Please let us know if you have recently taken on this role so that we can add you to our Carer's register. It also means that you will be eligible for a free flu vaccination over the winter to protect yourself and the person you are looking after...

If you would like any information or advice please contact Claire at VOCAL on 0131 663 6869

How to Register as a New Patient in the Practice

registration_formIf you live in the practice catchment area you are eligible to join the practice list. Please ask at Reception where you will be given the necessary forms to complete. You will be allocated one of the GPs who are taking on new patients.

However, if you wish to register with a specific doctor, we will do our best to accommodate you if the GP is agreeable.

You are free, however, to consult any of the doctors in the practice, irrespective of who you are registered with. All newly registered patients will be asked to have a consultation with one of the doctors or the practice nurse within one month of registering.

You will be asked to bring evidence of your identity with you such as a utility bill, driving licence or passport.
